Getting Started The getting started form gives you a brief overview of the steps and task ahead and your responsibilities in helping to make successfully achieve your goals.

Full Application This is the full application including getting started, application, signature card, & instructions. Use this to download everything you need at once, to get started. All information contained on the Full Application is confidential. It allows provides your customer service representative with the information needed to assess the types of services you will need to begin the credit repair process. It's very important that you answer the questions honestly and accurately.

Signature Card The signature card gives your customer service representative the authorization to request credit information from creditors.

Application Instructions This document instructs you how to fill out the application.

Regular Application This is your regular application. Use this if you already have the getting started, signature card, automatic payment authorization form, and addendums.

Automatic Payment Authorization Form You will need this form, if you are set up for a payment plan.

Retainer Application This is a special application. Use this form if instructed by your credit specialist. You will also need the getting started, signature card, automatic payment authorization form, and addendums.

Addendum One Please complete this form after you have filled out the application and are making your first payment. We want to make sure you understand whats next.

Addendum Two Please complete this form if you have had previous credit repair, used a counseling or debt consolidation company or have disputed items yourself.
